The Client
CAMDIAN is a Behavioral Health EMR/EHR platform that helps healthcare organizations manage clinical documentation, administrative operations, and revenue-cycle workflows in one place. As the platform matured, growing customer demand, evolving compliance obligations, and a constant stream of feature requests began to expose the structural limits of the original system.
The Challenge
CAMDIAN 1, the legacy platform, had become slow to change and risky to extend. Five problems compounded each other:
Development had lost momentum. Tasks stalled, feedback loops stretched out, and releases slipped, eroding stakeholder confidence and delaying client deliveries.
The architecture couldn't scale. Business logic was tangled across a monolithic codebase, so every new requirement touched code it had no reason to touch. Technical debt accumulated and performance degraded as functionality grew.
Requirements changed faster than the system could absorb. Behavioral health software evolves constantly, compliance updates, workflow refinements, customer feedback. Even minor changes required high effort and risky maintenance.
No comprehensive role-based access control. Clinicians, supervisors, billing staff, and administrators all need different levels of access to sensitive patient information. The legacy system had no granular, organization-aware permission model, a security and compliance exposure.
Poor user experience. Clinicians spend hours a day inside an EMR. Complicated multi-step workflows, inconsistent interfaces, and difficult navigation meant every unnecessary click was a direct cost to patient care.
Our Solution
Rather than patching symptoms, Zetaver ran a full platform assessment and rebuilt CAMDIAN1 as CAMDIAN 2, a ground-up modernization, not a version bump.
Modular, domain-driven architecture: We decomposed the monolith into independent, self-contained modules: client management, clinical documentation, workflows, claims, timesheets, pay cycles, access control, and more. Each module owns its own models, services, repositories, controllers, and routes. Adding a capability now means adding a module, not modifying a monolith.
Multi-tenant SaaS foundation: Redesigned database schema with proper relational integrity, isolated organizational data, optimized queries and indexing, and service/repository layers separating business logic from delivery concerns.
Configuration-driven workflows: Instead of hard-coding rigid flows, clinical documentation is defined as configurable workflows with steps, transitions, and role-based signature requirements. Organizations model their own approval chains without a code change.
Comprehensive RBAC and security hardening: Granular, action-level permissions enforced consistently across the API and interface. PHI encrypted at rest across client demographics, insurance records, service authorizations, and clinical signatures. Two-factor authentication with QR-based enrollment, token-based API authentication, and full audit trails.
Complete UI/UX modernization: A modern React single-page application with a component-driven design system: visual workflow builders, rich data tables, in-app digital signature capture, interactive dashboards, guided onboarding tours, and real-time notifications.
The Results
CAMDIAN2 is in production, serving healthcare organizations daily. Development responsiveness improved significantly, features that once required risky monolith changes now ship as clean, isolated modules with predictable delivery cycles. The multi-tenant, configuration-driven foundation means new organizations and new compliance requirements are absorbed through configuration, not redevelopment. Security posture transformed end to end: full RBAC, PHI encryption at rest, 2FA, and comprehensive audit logging. And clinicians work in a modern, consistent interface that reduces training burden and cuts the clicks between them and patient care.
